Investigation Summary
On June 9, 2004, Employee #1, a roofer, was installing new shingles on a roof while standing on an extension ladder. The ladder was not secured, standing on a scissor truck metal platform that was covered with shingle granular, and propped against a gutter at the eave of the building's roof. As Employee #1 installed roof jack brackets and attempted to place a walking board for fall protection, while handling a 2-inch by 6-inch by 8-foot wood plank, the bottom portion of the ladder kicked out and failed. He fell 18 feet to a concrete patio. Employee #1 was hospitalized at Miami Valley Hospital and treated for multiple bruises and contusions to the side of his head, as well as a fracture and misalignment of his lower back, which required surgery.
