Employee amputates foot in forklift accident

On April 24, 2004, an employee was operating a stand-up forklift truck stacking palletized materials in an enclosed lane, aisle way. Both sides of the aisle were stacked three high with palletized materials. The employee backed out of the lane with the adjacent lanes, stacked three high with palletized materials, blocking his view to the main aisle. As the employee exited the lane, another forklift was coming down the aisle The driver did not see the employee coming from the blind lane. The two trucks collided and the foot that was sticking outside the lines of the truck was amputated. The employee was hospitalized and treated for amputation. The cause of this accident was due to the employee going too fast, coming out of a blind lane, not blowing his horn, not stopping or slowing down at exit point of the lane, and having his foot outside the lines of the forklift truck.
