Employee is crushed by power industrial truck

On April 25, 2004, an employee was driving a powered industrial truck. When he got off the truck, he did not turn the engine off or set the parking brake. He either left it in gear or inadvertently bumped the shift lever, while departing the powered industrial truck. Once he realized the powered industrial truck was in gear and moving, he attempted to stop it. He was struck and crushed against a column as he was moving along side of the machine and reaching for the controls of the powered industrial truck. He appeared to be reaching for the keys or shift lever to take the machine out of gear when he was struck.
