Investigation Summary
On October 25, 2004, an employee was on a three man crew assigned to clear brush from the natural gas pipeline. The employees began to cut a tree down when a coworker in the Hydro Ax thought that the tree was going to fall into power lines. The coworker on the Hydro Ax moved ahead to push the tree in a different direction using the rotary ax attachment. The coworker did not see the employee when moving the Hydro Ax ahead; he caught the employee between the tree and the tire of the Hydro Ax. The employee was killed.
