Investigation Summary

Investigation Nr: 200022929
Event: 04/13/2004
Employee Is Killed in Fall from Platform of Picker

On April 13, 2004, an employee was approximately 24 ft in the air on a Cleco High Order Picker fork lift truck. He was found on the floor bleeding profusely from his head. He was wearing a safety belt but no lanyard. Initial observation was a fall. However the medical examiner's initial report was cerebral trauma. It appears that a package of clothing had fallen from above his work level and struck him in the head as he was retrieving a package from 24 ft level. The overall height of stacked package racks was 65 ft. Coroner's finding was that the death was caused by the fall.
