Investigation Summary
At 3:00 p.m. on May 5, 2005, Employee #1 was cleaning/repairing jewelry using denatured alcohol. There were jewelers' torches in the area, one having the valves in the open positions. The denatured alcohol ignited, causing the one-gallon container to erupt in an explosive manner, resulting in Employee #1 being set on fire. He was taken to the hospital, where he died nine days later. .
