Employee Struck and Killed by Falling Object

At approximately 12:30 p.m. on November 2, 2004, an employee of a cargo company was operating a forklift and Employee #1, an independent trucker, were loading galvanized pipe onto a flatbed. As the last bundles of pipe were being loaded, Employee #1 suddenly directed the forklift operator to stop, causing one of the bundles to slip off the forks and fall on Employee #1, fatally injuring him.
