Employee is killed when pressure released from pump

At approximately 6:30 p.m. on September 26, 2004, an employee and a coworker were preparing Pump Unit Number 1 to dispose of salt water back down hole. At approximately 6:40 p.m., the operator released the clutch for the pump, and the employee closed both circulation valves. The side of the local shop made the compression dampener split open, releasing internal pressure, pushing the employee against other equipment and killing him.
