Employee killed after fall from platform

On June 2, 2004, Employee #1 was working at a construction site. He was located on a form-wall system that was 52 feet from ground level. As he stepped down to a wooden platform, 0.75-inches thick, the wood broke and Employee #1 fell through the platform. He hit and bounced off another platform that was 32 feet above ground level and struck the ground head first. Employee #1 was killed from massive head and multiple internal and external injuries.
