Employee injured when struck by falling object

At approximately 8:45 p.m. on March 16, 2004, Employee #1 and a coworker were pouring concrete on the top floor using a concrete placement boom. One of the drive links broke causing the tip section to fall and strike Employee #1 who was holding the flexible hose. He sustained moderate injuries to his shoulder and back and was hospitalized.
