Investigation Summary
Employee #1, a foreman, was working from a scaffold platform and was installing Eldorado Stone around the face of the garage of a residential home. He had three sections of fabricated frame scaffold erected across the front of the garage and each section was 6 feet high. While he was walking on the scaffold platform, he lost his balance and fell approximately 6 feet onto the concrete floor. He struck his head on the floor and suffered contusions, lacerations, and a concussion. He was hospitalized with his injuries.
