Employee Is Electrocuted While Connecting Transmitter

On August 19, 2004, Employee #1 was installing a high voltage radio frequency transmitter. A new 199 ft high radio tower was at a remote site. He was connecting the incoming 230 volt three phase power sources to the transmitter. He was not wearing personal protective equipment, such as a hard hat, shield, and insulating gloves. He was not using nonconductive tools and was working by himself. The transmitter was to step up the incoming power to 10,000 dc volts. He was found electrocuted. It appeared he was testing a circuit through the apparatus as one of the leads was off. The 230 volt three phase motor was running. Employee #1 may not have been qualified for this type of work.
