Investigation Summary
At approximately 9:00 a.m. on August 24, 2005, an employee and a coworker were loading a Ford F-650 flat bed truck with OSB plywood for delivery. The employee was placing the wood skids onto the bed of the truck, and the coworker was operating the forklift. While the employee was in the process of adjusting the skids on the truck, the forklift operator backed the forklift up a short distance while simultaneously lifting the load of OSB plywood to the approximate loading height of the truck. After the operator lifted the load he was no longer able to see the employee who was adjusting the skids. The operator moved the forklift forward to begin placing the load onto the truck, when he heard a moaning sound. Another coworker near the area where the truck was being loaded started shouting at the operator to back up. The operator of the forklift panicked and began to lift the load before he backed the forklift away from the truck. The employee that was adjusting the wood skids on the bed of the truck had been caught between the forklift's load of OSB plywood and the stationary truck bed. The employee walked a few steps away from the truck and collapsed to the ground. He was transported to the hospital, where he died during surgery.
