Employee Is Killed in Fall from Roof

On August 16, 2005, Employee #1 was part of a crew engaged in constructing a standing-seam metal roof on a systems-engineered metal building. The employer used a scissor lift from inside the building to access the roof. The path of the scissor lift was obstructed and the employer positioned the lift perpendicular to the purlins (horizontal timbers supporting rafters of a roof). The work platform was approximately 51.5 in. below the roof surface (42.5-in. high guardrail system plus the 9-in. high purlin). Because of lift platform's position, the employees had to first disconnect their fall protection lanyards from the anchorage point before entering the lift. As Employee #1 walked to the area of the scissor lift, he fell through the roofing insulation and landed approximately 28.5 ft below on the concrete foundation. Employee #1 was killed.
