At about 11:15 a.m. on February 22, 2005, Employee #1, of Peak USA, was dismantling and lowering the top substructures of an oil drilling rig. The 80-ton crane used to lift the substructures began to tilt forward and lose the load. Employee #1 was standing next to the front right track of the crane.
