Employee Dies from Fall

On November 1, 2004, Employee #1 was standing on a wooden pallet attached to the forks on a Caterpillar Forklift truck. Employee #1 was retrieving merchandise from a warehouse storage shelf, located 18 feet above the concrete floor. Employee #1 fell backward, struck his head, and died from his injuries. He was not wearing any fall protection equipment.
