At approximately 11:00 a.m. on June 14, 2004, Employee #1 collapsed while standing on the bank of an excavation taping the end of a PVC pipe. Employee #1 was pronounced dead at 12:02 p.m. at a local hospital. The cause of death had not been determined by the medical examiner at the time of the original narrative.
