Investigation Summary
On May 31, 2005, Employee #1 and two coworkers were preparing to remove dead and dangerous trees adjacent to an electric power line. Employee #1 was using a rope to guide the direction of the tree to be felled. Another employee was cutting the tree trunk. Simultaneously, as the tree was falling, Employee #1, who was using the rope, turned and ran into the direction of the falling tree. Employee #1 was crushed and died from complications after being struck by the tree.
