Employee Falls from Ladder and Fractures Hip

At approximately 9:30 a.m. on January 19, 2007, Employee #1 was going to use a 20 ft extension ladder to trim palm trees at a miniature golf course. He extended the ladder to 16 ft and leaned it against the tree to be cut. The feet of the ladder rested on a concrete surface and he had his hand saw in his pocket. He started to climb the ladder with both hands on the rungs. He stated the ladder had not been tied off because he could not reach the branches he was going to cut. He started to climb down the unsecured ladder and it started to sway. He fell approximately 6 ft off the ladder and landed in a water pond between holes number 5 and 6. He fractured his hip and was hospitalized.
