Employee's Toe Is Amputated When Crushed by Forklift

At approximately 7:30 p.m. on October 21, 2006, Employee #1 was working at a food warehouse. He was operating a Pallet Jack, Number 211, Model PMWR 30, Serial Number PMWR 33193, going backward in a central aisle. As he was making a right turn to enter into aisle Number 4, he struck another fork-lift, a Mitsubishi, Number 211, Model FDC 25A, Serial Number A3BC 210421, which was parked in the aisle. He put his right foot out of the running lines of the pallet jack and lost control of the pallet jack. His right foot was caught between the pallet jack and the fork-lift and was crushed. He was hospitalized and his big toe and second toe were amputated.
