Investigation Summary
At approximately 11:38 a.m. on September 18, 2006, Employee #1 of IAP Worldwide Services, a U.S. Army contractor, and another contractor employee, entered Building #29 to check the heating/ventilation/air conditioning unit. Only the front part of the building was fully illuminated due to energy conservation. Employee #1 and the other contractor employee went to the back of the building where the unit was located. As Employee #1 was checking the reading on the unit, he stepped slightly to the side to see the readout on the unit and fell through an unguarded floor opening. He fell 8 feet onto the cement floor below. He received multiple injuries that required hospitalization for 22 days.
