Investigation Summary
At approximately 10:50 p.m. on August 29, 2006, Employee #1 was removing an accumulation of soft chocolate product that had built-up on a coating machine. His hand became caught between a metal bar and a guard. Four of his fingers were amputated. He was hospitalized at Loma Linda University Medical Center.
