Investigation Summary
At approximately 12:30 p.m. on September 19, 2006, Employee #1 was hanging drywall at a work site in Riverside, CA. He was working from a 19 1/4 in. high, 27 1/2 in. square water heater platform when he tripped and fell over. He was holding a utility knife in his hand and as he tried to break his fall with his arm, he accidentally stabbed himself in the neck. At least three coworkers called 911, and the fire department and paramedics arrived within minutes. The fire department found Employee #1 lying down on the drywall and covered with blood. He had a 1 in. to 2 in. laceration on his neck that appeared to involve his jugular vein and/or carotid artery. Employee #1 had lost a significant amount of blood, was having difficulty breathing, and was not fully conscious. He was provided emergency medical care and transported by the ambulance to Riverside Community Hospital, where he died three days later.
