At 3:30 p.m. on August 22, 2006, Employee #1 was plastic wrapping boxes that were on a pallet jack. One of his legs became caught on the forks of the pallet jack, sustaining a fracture to the lower part of his leg. Employee #1 was hospitalized and treated for his injuries.
