Employee Is Injured While Operating Stand-Up Type Forklift

At approximately 10:50 a.m. on August 22, 2006, Employee #1 was operating a Crown stand-up type forklift, Model Number RC3020, Serial Number 1A276091, in the receiving department of a food distribution warehouse. As Employee #1 was driving the stand-up type forklift backwards, he made a U-turn and lost control on the forklift. He placed his left leg out of the running lines of the forklift and hit a storage rack. His left foot got caught between the storage rack and the forklift, causing a serious crushing injury to his left ankle Employee #1 was hospitalized, where he was treated for his injury that required the big toe on his left foot to be amputated.
