Employee Amputates Leg While Parking Forklift

At approximately 10:40 p.m. on July 26, 2006, an employee was operating a stand-up type fork-lift, Raymond, Model Number-R40C40TT, Serial Number R40 0204029, in the North Dock Area, Building Number 3. The employee was parking the fork-lift while driving backward. He lost control on the forklift and accidentally he put his left leg out of the running lines of the forklift and hit a bollard post, yellow color. His left lower leg got caught between the bollard and the forklift. The accident caused crushing injury to his left lower leg which required hospitalization for two weeks and his lower leg had to be amputated at the hospital.
