Investigation Summary
At approximately 10:31 a.m. on May 30, 2006, Employee #1, a forklift operator with Champion Lumber Company, Riverside, California, was driving a Hyster Model H80XM lift truck forward while carrying a 8,312-pound load of green Douglas fir lumber high in order to see where he was going. He braked hard to avoid hitting a lumber stack. This caused the lift truck's center of gravity to move outside the stability triangle, resulting in a tip over forward incident and the lumber fell off the forks. Employee #1 was seat-belted and remained in the vehicle. When the lift truck fell back hard on its rear wheel, Employee #1 sustained a compression fracture to his lower back. Employee #1 was taken to the Riverside Community Hospital, Riverside, California, where he was evaluated, admitted and hospitalized for thirteen days.
