Investigation Summary
At approximately 5:45 p.m. on May 18, 2006, Employee #1, a forklift operator, was unloading a truck trailer, when his right hand was caught between the frame of the forklift and the rear door of the truck trailer, crushing two of his fingers on his right hand. Employee #1 transported to the hospital, where he was treated and released.
