Investigation Summary
At approximately 10:15 a.m. on December 17, 2005, Employee #1 was working as a regular employee of Quality Shutters, a manufacturer of wooden shutters in Riverside, CA. As Employee #1 was cutting a piece of wood, his left hand came into contact with the unguarded blade of a Powermatic table saw. He lacerated three fingers on that hand, and he was hospitalized.
