Investigation Summary
At approximately 6:10 p.m. on October 24, 2005, Employee #1 was assigned as a loader for outbound freight. Employee #1 realized that the tractor trailer had moved forward and was not secured or restrained before the loader started to load. Employee #1 was injured when he jumped off of a stand-up forklift which was partially in the back of the tractor trailer. Employee #1 sustained a left radius fracture, small and ring finger fractures and was hospitalized for eight days. ,
