Employee Is Injured by Falling Bundles

At approximately 3:15 p.m. on October 7, 2005, Employee #1 was operating a pallet jack. As he passed by a stack of bundles of empty bags for stucco, stacked four bags high, two bundles from the top fell off the stack. One bundle fell on the handle of the pallet jack and rolled over on Employee #1's left leg. His leg became pinned between the bundle and the pallet jack, causing injuries to his femur. The handle of the pallet jack struck his forehead. Employee #1 was hospitalized for surgery on his femur and stayed for two days.
