Investigation Summary
At approximately 9:00 p.m. on June 23, 2005, Employee #1 was working as a temporary employee. With his supervisor working next to him, he was bolting a deenergized uncoiler machine to the floor after the machine had been moved to a new location. One of the machine's components was interfering with the installation of the bolts. The supervisor had placed a detachable metal handle onto a spindle of the uncoiler so that the component could be repositioned. Nonetheless, the supervisor was unable to move the uncoiler component. A coworker, a regular employee at the accident site, was able to move the uncoiler component out of the way. When the coworker finished moving the component, the metal handle was left on the uncoiler spindle. As Employee #1, in a kneeling position, tightened the bolts with a wrench, his supervisor was on another side of the uncoiler hitting a bolt into the floor. The vibration caused the metal handle to fall off the spindle. The eight pound metal handle struck the tip of the small finger on Employee #1's left hand, crushing it against the floor. Employee #1 sustained an open fracture. He was transported to Inland Occupational Medical Clinic and subsequently transferred to Riverside Community Hospital, where he was admitted. He was hospitalized for 2 days.
