Investigation Summary
On April 9, 2005, Employee #1 was working for a frozen Mexican specialty foods manufacturer. The food products are cooked, formed by hand and automated. The product is fried and quick frozen. The product goes from fabrication by a long conveyor to the fryers and freezers. The employee was cleaning the sprocket chain drive system, between Line 1 and the number 2 Fryer. The chain drive has an on/off switch and an emergency switch. Both switches need to be turned off prior to cleaning the guard covering the sprocket chain drive. If the emergency stop is not turned off, the belt can be started by turning on the switch at the transfer conveyor belt between the fryer and the freezer. The conveyor belt is cleaned by turning the switch on/off to jog it. A coworker was cleaning the conveyor and turned the power on. The employee's fingers were caught in the moving chain/belt system. He amputated his right index and middle fingers, at the first joints. He was taken to the hospital, where surgeons reattached the severed portion of his index finger. Employee #1 was hospitalized.
