Investigation Summary
At approximately 1:46 p.m. on March 31, 2005, Employee #1 and a coworker were loading a date palm tree onto the bed of a Stinger Truck-Mounted Crane, 4700 Series, Model Number 3T4792, Serial Number 49004021558, Unit Number 144. The date palm tree was standing vertically on the bed of the truck mounted crane. Employee #1 and his coworker proceeded to the location where they were going to place the date palm tree inside a 7-ft trench. As Employee #1 and the coworker approached the trench, Employee #1 climbed out the cab of the truck mounted crane to check the trench. Employee #1 was positioning the truck when the tree started swinging, causing the boom to swing. The truck started to tip over, and Employee #1 climbed out of the cab of the truck and started to run. As Employee #1 ran, the truck mounted crane boom struck him in the head. Employee #1 was approximately 30 ft from the cab of the truck. Employee #1 was taken to the hospital, where he died from his injuries.
