Investigation Summary
At approximately 4:00 p.m. on February 14, 2005, a coworker was operating a Crown RC3020 stand-up forklift, Serial Number 1A284073, with pallets of goods on the forks. As Employee #1 approached the forklift, the coworker asked her if she wanted a ride back to the main building. Employee #1 accepted and got into the operator's cage of the forklift with the coworker. As they rode, the load on the forks began to shake. Employee #1 lost her balance and fell off the forklift and her left leg was run over by the forklift's tires. She was transported to Arrowhead Regional Medical Center where her leg was amputated at the knee and she was hospitalized for 6 weeks.
