Investigation Summary
At approximately 5:30 a.m. on December 31, 2004, Employee #1, a forklift operator, was working as a temporary employee for his primary employer Helpmates Staffing Service in Rancho Cucamonga, California at a jobsite for the secondary employer, Commodities Logistics West Inc. located in Mira Loma, California. He was operating a forklift (RC 3020-40, Serial Number: 1A221754) and ran into the T-rack pole at aisle 6800 in order to avoid hitting another vehicle that was stopped at the aisle. His left foot was inadvertently extended down and outside of the operator's platform, when the back of the vehicle struck the metal pole and his foot was smashed by the impact created by the rear of the vehicle and the pole of a T-rack. Employee #1 suffered a complex laceration and was transported to US Healthworks, where his condition was evaluated. Then, he was referred to Chino Valley Hospital, where he underwent surgery and was hospitalized.
