Investigation Summary
At approximately 9:00 a.m. on December 21, 2004, Employee #1 was driving a hopper truck-trailer, when a tree that was being felled by a crew across the road fell on the cab of the truck. The tree crushed Employee #1 inside the cab. Employee #1 was pronounced dead at the accident site. Employee #1 was a regular employee of the company.
