Employee Is Killed When Arm Caught in Conveyor

At approximately 6:40 a.m. on December 7, 2004, Employee #1, a machine operator and maintenance worker, was checking all the machines and equipment in a facility that recycles condemned automobiles. He noticed that a piece of metal debris was stuck to a rotating magnetic head pulley on the second level, located in the downstream area. He then climbed the standard railing and tried to remove the stuck piece off the magnetic head pulley with his right hand, but his right hand got caught. He was then pulled into the running conveyor belt and magnetic head pulley. Employee #1 was killed.
