Event: 11/22/2004
Employee Amputates Finger in Chain Sprocket

At approximately 3:15 p.m. on November 22, 2004, Employee #1, a foreman, was washing down a running portable electric-powered belt conveyer with a hose. Employee #1 slipped on wet concrete as he reached to remove a potato that was on top of the conveyer's chain and sprocket drive guard. As Employee #1 fell towards the conveyer, he reached for something to break his fall with his right hand. When he grabbed the top of the chain and sprocket guard, his gloved-hand was drawn into the in-running nip point. A coworker shut down the conveyer and had to remove the guard and detach the chain to extract Employee #1's hand from the nip point. After providing first aid, the coworker drove Employee #1 to the hospital where he was admitted and hospitalized for two days. A portion of both Employee #1's right ring and little finger was surgically amputated because of extensive damage.
