Investigation Summary

Investigation Nr: 202439790
Event: 08/24/2004
Employee's finger is amputated while banding load

At approximately 9:00 p.m. on August 24, 2004, Employee #1, a laborer with T. M. Cobb Co., was using a FROMM battery-powered plastic strapping tool (Model Number P321) to band a pallet of wooden doors together for shipping. As he was placing an edge protector on a door under the plastic banding strap with his left hand, he activated the tool, and his left index finger became caught and squeezed between the banding strap and the edge protector. When Employee #1 jerked his hand to free his finger, the tip of the finger was pinched off. He was treated at the S.S. Healthworks Clinic in Riverside, and then hospitalized at Loma Linda University Medical Center for orthopedic surgery.
