Employee's Finger Is Amputated with Saw

At 8:52 a. m., on August 13, 2004, Employee #1 was working maintenance in a warehouse for his employer, a retailer with a warehouse and distribution center in California. Employee #1 was cutting melamine board with a saw. Employee #1 slipped on saw dust on the floor causing his hand to make contact with the unguarded saw blade. Employee #1 sustained an amputation of the left index finger and deep lacerations of the middle left finger. The Fired Department/Paramedics arrived at the warehouse and transported Employee #1 to the Loma Linda Medical Center for treatment and surgery. Employee #1 was hospitalized for three days. At 3:20 p. m., on August 13, 2004, the employer reported the incident to the San Bernardino District office.
