Employee Suffers Amputation When Struck by Falling Object

At approximately 7:25 a.m. on July 19, 2004, Employee #1 was removing two extensions used for the forklift from under a metal pallet (119-in. by 48-in.) with a shed (125-in. by 88-in.) placed on the pallet. A coworker had lifted up the load from one side with a forklift so Employee #1 could remove the extensions. Employee #1 had removed one extension, and, as he tried to pull the other one, the pallet slipped off the forks and fell and crushed his right thumb between the pallet and the extension. Employee #1 suffered a severe laceration on his right thumb, and part of his thumb had to be amputated. Employee #1 was hospitalized.
