Employee's Hand Caught Between Belt and Rollers

At approximately 3:00 a.m. on July 2, 2004, Employee #1 was performing maintenance on a running conveyor. Employee #1 removed the Plexiglas guard, the tensioning roller and scraper blade assembly from the Die Cut Line, and took them back to the maintenance shop for repairs. After completing the repairs, Employee #1 took the tensioning roller and metal scraper back to the line. Employee #1 installed the tensioning roller and the metal scraper and then started the conveyor belt to check for proper belt tracking. Employee #1 started to adjust the metal scraper, with the belt running, and without the Plexiglas guard in place. During this process Employee #1's left hand was caught between the tensioning roller and the main cutter drive roller. Employee #1's left wrist was fractured and he was hospitalized for one day.
