Investigation Summary
At approximately 2:00 p.m. on December 10, 2003, Employee #1 was operating an electric powered "stand in" material handling device, moving palletized perishable goods within the warehouse. As Employee #1 was exiting an aisle, he drove through some standing accumulation of water on the floor. The wheels lost traction with the concrete floor and began to slide towards the steel poles that were imbedded in the concrete floor providing protection from the vertical storage rack loaded with goods. Employee #1 extended his left leg towards the steel pole in an attempt to slow down. Unfortunately, the force of the device was greater than what was anticipated, his left ankle was pinned between the device and the steel pole when the two objects collided. He sustained a compound fracture of the left ankle.
