Investigation Summary
On August 10, 2004, Employee #1 was changing ballast on an overhead fluorescent light. The power was not turned off and not locked out. He went to reconnect the wires to the new ballast, and he contacted the hot wire on the web of the right hand between the thumb and index finger and was electrocuted.
