Employee is caught between machine rollers and killed

On May 4, 2004, an employee of a plastic manufacturing company removed plastic wrap from the deflate rolls on a film extrusion line, and was caught between the bow roll assembly and the deflate assembly as the elevator moved upward. The employee was positioned at the racking section of the extrusion line when the incident occurred. The employee was pulled into the machine and killed by asphyxiation.
