Employee Is Killed in Fall from Deck

At 3:35 p.m. on January 19, 2005, Employee #1, of the Don R. Fruchey, Inc., steel fabricators, was installing metal decking 36 feet above the lower level without having fall protection standard for working at heights of over 30 feet. As he was working, the last two pieces of metal sheeting, still loose, shifted on the structural beam beneath him, and he fell through the opening of the metal decking. He struck a metal beam on the way down to the lower level. The weather could have been a contributing factor. At the time, the temperature was 32 degrees F, with a 16 mph wind and snow and mist. There was ice on the metal decking.
