Investigation Summary

Investigation Nr: 200993681
Event: 07/14/2004
Employee killed when caught between truck cab and trailer

At approximately 11:15 a.m. on July 14, 2004, a truck driver was backing a yard truck up to an empty semi-trailer so that trailer could be loaded at an empty loading dock. There were no witnesses to this accident. He backed the truck under the trailer but the kingpin and the fifth wheel did not connect. He may have thought that the kingpin was attached to the fifth wheel and left the gear shift in reverse. The fifth wheel was several inches behind the kingpin when he was discovered. He climbed out of the cab and onto the running platform to hook up the air brake hoses. Apparently, the truck moved backwards, trapping him between the rear of the cab and the front of the trailer. He was found approximately 15 minutes later by another truck driver. The co-worker placed the truck in neutral and the employee was released from his position. CPR was administered and he was transported to an Indianapolis Hospital where he died two days later.
