One Employee Killed and One Injured When Structure Collapses

On April 19, 2005, Employee #1 and Employee #2 were removing jacks that were supporting concrete panels. Employee #1 climbed a ladder and removed a nut from a bolt that held the jack in place. He came down from the ladder and helped Employee #2 remove the nut securing the lower foot of the jack and lowered the jack down. After removing the jacks on the south wall, Employee #1 climbed an extension ladder and removed the nut from the bolt that was used to secure the top of the jack to the front center wall panel. After removing the nut he came down off the ladder. The wall panel was not secured at the top and it fell over causing other wall panels and a section of the roof to collapse. Employee #1 received a fractured back and was hospitalized. Employee #2 was killed.
