Summary Nr: 202344354Event: 03/02/2004Employee Killed And Other Injured In A Crane Accident
On March 2, 2004, Employees #1 and #2 were stripping steel forms from concrete pillars. The forms were 18 ft long and weighed one thousand pounds. The rigging that was being used to lift the forms was defective. When the form was being laid down on the ground, it rolled out of the deformed hook on the rigging. The form fell free on to the employees killing Employee #1 and severely injuring Employee #2.
